Any person wishing to apply for a certificate of acceptability in respect of a milking shed shall apply for it in writing and in his application furnish the following information: 

• The name, address and telephone number of the applicant;
• A title description of the premises; and
• An indication of the number of staff that will be employed, or that will probably be employed, and the maximum number of animals that will be milked.

Applications is to be submitted to the local authority in whose area of jurisdiction the milking shed is located or will be erected, together with-

• A site plan with north indicated and with an indication of all adjacent and bordering buildings and their uses;
• A layout sketch to a scale of 1:100 of all the milking shed facilities namely:
  • Milk container
  • Milking machine
  • Bulk farm tank
  Consisting of a ground plan, a sectional view and a vertical elevation
